Bayshore Medical Personnel is seeking Primary Care Nurses (PCN) to provide service to our client in a primary healthcare setting in Gagetown, NB..PCN’s are responsible for the provision of primary health care services to members, and to provide patient assessment, screening, healthy lifestyle support, education, and chronic disease management with a goal of improving health outcomes and facilitating access to health services.They function as a member of a multi-disciplinary health care team to develop and implement care plans, and provide patient follow-up.
